Leadership Permission Form

If your son/daughter is selected to the Eagle Bay Camp Leader In-Training (LIT) or Servant Leadership program (SLP) for this season please be aware of these permissions that need to be signed and sent in with the application. While the camp strives to provide supervision of the LITs during camp activities, they may have unscheduled free time on the weekends from Saturday after 11am until Sunday at 11am. We require your permission to allow your son or daughter to possibly leave the property of Eagle Bay Camp during this time. They may, for example, wish to go to the EBC Mercantile down the road. (They would be accompanied by a coordinator.)

We would also like your permission for your son or daughter to leave the camp property for occasional off-site team building experiences. We may also be taking the LIT 1s on a short trip of 1 night away from camp and the LIT 2s may be going away for a one to two night out-trip as apart of their program. For these LIT activities, they will be under the direct supervision of the LIT Coordinators. For all of the aforementioned situations, we will need your written permission for your son/daughter to leave the camp property. The permission form below must be filled out and submitted by the application deadline of March 15th for all LITs & SLPs (both July and August).
